REPLACEMENT OF DRIVERS LICENSE
i. A Licence is replaced as a result of loss or damage.
ii. Applicant must report to the nearest Police Station and obtain extract of the original copy of which should be attached to the request for replacement.
iii. Applicant should swear an affidavit stating the facts of the loss, damage or defacement and
iv. pay the prescribed fee of (currently N3,000.000) to the Licensing Office, complete Form RS15R and submit for processing.
